- Are you a student registered in a regular program at the University of Ottawa and a member of Local 2626 of the Canadian Union of Public Employees (CUPE)?
- Do you work at the University of Ottawa as a Teaching Assistant, Demonstrator, Lab Monitor, Tutor, Corrector, Proctor or Research Assistant,
- Do you pay union dues to CUPE (see code 158 on your pay stub) and are therefore bound by the Collective Agreement signed by your association and,

- Did you receive, in lieu of agreement with the University, a signed copy of the form (S form) sent to Human Resources to activate your pay?
